Canadian Armed Forces personnel

If you are a member of the Canadian Armed Forces and see a military dentist regularly, we offer dental hygiene (teeth cleaning) appointments at our office.

Please note that panoramic X-rays are not available at our office.

We require a copy of the signed pre-authorization form and a picture of your Medavie Blue Cross insurance card to be emailed to us before your appointment.
